Electronic Re-certification Packet Submission - Claiming Points
PLEASE NOTE:
We have recently been made aware that the re-certification form instructions are in conflict with regards to submitting your supporting documentation and whether your Professional Development Report documentation should be combined with your Specialty Documentation. As long as your documentation is submitted, either combined or separately, it will be counted. We only need one copy of your documentation.
ADDITIONALLY, we were also notified that some of the form fields do not allow for half points. Please make sure your Declaration Page (09-0612F) accurately reflects the points you are claiming. If a form field does not allow you to enter your half points, round in either direction and make a note on your Declaration page of the section in which you were trying to claim the points and your accurate points.
We will be making the Re-certification Committee Members aware of this issue and will be requesting that special attention be paid to half points. We deeply apologize for these issues.

For the 2019 Re-certification Year, submitted in 2020, you will be able to fill out and submit your entire Re-certification Packet electronically. The only .pdf forms you will have to complete and scan are your Re-certification Declaration form (09-0612F) and your signed Rules of Professional Conduct (09-0001F). You will also need to scan your supporting documentation for Sections II, III and specialty sections, as applicable.
THERE IS A CHANGE FOR THIS YEAR, AS YOUR SECTIONS II AND III DOCUMENTATION MUST BE SCANNED SEPARATELY FROM YOUR SPECIALTY DOCUMENTATION.
As you will be filling out the information included on the Re-certification Professional Development Report (09-0601F) and, if needed, the Specialty Professional Development Report (09-0611F) via the electronic forms linked at the bottom, there is no need to also fill out the .pdf forms located on our Forms page. You also do not need to include the pdf versions of the 09-0601F or 09-0611F as part of your documentation packets.
Before proceeding, please take a moment to review the general steps for this electronic submission process listed below.
Electronic submission of your Re-certification Paperwork is optional, and you always have the option of mailing your Re-certification Paperwork to the ABC Registrar’s Office at PO Box 1358, Palmetto, FL 34220.
Step One
Read the current Recertification Instructions for guidance on how to complete all necessary forms.
Step Two
Review the current Recertification Point Structure (09-0608S) to see where you are claiming points and to assist in gathering the required documentation.
Step Three
Complete the Recertification Declaration Page (09-0612F), including your signature. Also sign the Rules of Professional Conduct (09-0001F).
Step Four
Compile all required documentation to support the completed forms. Place your documentation into 2 sets: 1 for your Specialty documentation and 1 for all other documentation. You will submit your main documentation with your Professional Development Report and your Specialty documentation with your Specialty Professional Development Report.
Step Five
Scan your Recertification Declaration page (step 3), your main documentation (step 4), and your signed Rules of Professional Conduct (step 3) into one (1) file. This file must be in .pdf (preferred), .doc or .docx format. The maximum file size is 10MB, so it is recommended that you scan each page at no more than 150 dpi. Please save this file as YourLastName_Recert Documentation.

Scan your Specialty documentation (step 4) into one (1) file. This file must be in .pdf (preferred), .doc or .docx format. The maximum file size is 10MB, so it is recommended that you scan each page at no more than 150 dpi. Please save this file as YourLastName_Specialty Documentation. If not included, your points will not count.
Step Seven
Fellows Only: Download, complete, sign and scan the Proficiency Testing Certification Form (09-0701F) into a second file.
Step Eight
There are 2 buttons below: “Access Professional Development Electronic Form” and “Access Specialty Professional Development Electronic Form”
Once you click on the button, you will be taken to the appropriate form. Complete the form, attaching your Documentation Packet and your Proficiency Testing Certification Form (if appropriate) at the end.
Once the Professional Development Form and Documentation Packet is submitted you should be directed to a Thank You page. If you are not directed to the Thank You page, or you are directed to another page, please email [email protected] to confirm your electronic submission was received. Upon successful electronic submission of your recertification packet, you should receive an email confirmation.
If you also need to report on the Specialty Professional Development Report, then click on the “Access Specialty Professional Development Electronic Form” button. Complete the form, attaching your Documentation Packet at the end. You should once again receive a Thank you page at the end.
After both submissions you should also receive an email with your information. If you need to edit your submission (either one) please click on the “Edit Submission” link on that email. If you do not receive an email, please email [email protected] for a copy.
Known Compatibility Issues
There are known issues using the Chrome browser with Windows, therefore if you are using a Windows computer, we suggest that you use a browser other than Chrome. There are known issues using the Firefox browser with Macs, therefore if you are using a Mac computer, we suggest that you use a browser other than Firefox.
There have also been issues with this submission process using some government computer systems, mainly due to the firewalls in place on government networks. If you work for a government agency, you may be better off completing this online submission using your home computer rather than a government network.
